Our Lincoln Park office is located near the intersection of West Diversey Parkway, N. Clark St., and Broadway in Chicago. From I-90/94 east (Kennedy Expressway), take exit 46A for California Ave. From I-90/94 west, take exit 46B for Diversey Ave. Our office can also be reached from Lake Shore Drive by taking either the Fullerton Ave. exit or the Belmont Ave. exit.

Metered street parking is available along Diversey Parkway and other area streets. A public parking garage is also located across the street from our office.
Public Transportation
Our Lincoln Park office is served by CTA bus routes 22, 36, and 76. We are also located 1/2 mile away from the Diversey Brown and Purple line station.

Our downtown Chicago office is located in Two Prudential Plaza, just east of the Loop and Michigan Avenue. From I-90/94 east (Kennedy Expressway), take exit 51C for East Washington Blvd. From I-90/94 west, take exit 51A for Lake St. From Lake Shore Drive, take the Randolph St. exit.

Our downtown Chicago office is served by a number of CTA bus and train routes converging in the Chicago Loop area, as well as the South Shore Line's Randolph St. station.

Payment Information
Payment for services is due on the day of service by cash, check or credit card. You may request a receipt for your payment in the form of a superbill. A superbill is an official receipt of services that you may submit to your insurance company. Your insurance company may partially or fully cover the registered dietitian services you received and directly reimburse you.
